Next, process of elimination.

A thin wooden wedge is required :)

Lets get the derailleur on the top sprocket and wedge the body so it can't loose tension. Most derailleurs have a gap in the body when on the top sprocket, stick the wedge in the gap. It won't harm to screw the 'H' screw right in as well, so that derailleur isn't going nowhere!

Stomp it up a hill.

See what happens..........
 
I hope I don't jinx things with this post but I think I've found a fix for it! I suspected the cable guide was causing extra friction and wasn't helping, so I cut up one of those plastic cable guides that screws into a bottom bracket shell to wedge between the existing cable guides and routed the rear cable through that, went for a ride to work and back today and no gear jumping even when I stand on the pedals! Fingers crossed that's it sorted permanently, cancel the hunger strike oonaff!
