Do you have any stems that use recessed bolts? I’ve seen two bikes so far with recessed stem bolts that use m5 washers under the bolts for the steerer side clamp.
They were both aluminum stems if that matters? Not sure if this is “a thing”.

That was my guess*, I just couldn't remember what they are called (well not the brand names anyway, not my thing, I looked up cheap centre pull brakes. )
Though I was thinking even cheaper and the side pull ones. Or whatever they call them.

*bundled with the mud guards, reflector stuff.
