Do you use a torque wrench for the M6 bolts on the Hollowtech 2 crank arms? If so, do you go all the way to the recommended setting? My old cranks splines are shot, I'm wondering if this is to do with too much or too little torque I applied previously :?

Both, too little and the crank will move/fret against the steel spline. Too much, you'll deform the spline interface and it won't mesh properly (especially if you remove and refit).

You'd have to go really mad, way outside of tolerance to do that though........

I have seen HTII tightened until the split in the crank is closed (no spacer either!!)
 
The correct torque is two knuckles tight; that is to say that only two of your knuckles should turn white.

The lawyer tab spacer thing should in all cases be removed
