Nope i did have the part 6 but it just keep on sliding up and down the shaft.

Now I think thats the problem, the collar is not tightening when i turn the bolt.

Any suggestion on how to tighten the collar ?
 
Pretty sure it has a step in the bottom so the shaft will only go so far in before bottoms and then the taper starts to bind / do its job as you continue to tighten the bolt. Will check and report back ;)

The bush goes onto the shaft and stops when it bottoms on the flange. When you tighten the bolt the taper pulls in and clamps the bush onto the shaft (that's why it's split).

That's why I posted above that it should be assembled dry - it needs friction to tighten.

I reckon if your bush slides all the way up the shaft then the flange is knackerd / broken off :-(
