I remember a few years ago reading an article in MBR (I think) about making sure the stem clamping area and brake/shifter claps were free from burr's or ridges. They advised checking the inside of the clamp and using fine needle files and sandpaper to make sure it was all smooth. Particularly around the edges where burr's can be left after machining.
