Shimano 737 front shifter labeled as 565, lx?
Anyone seen anything like this before? Any ideas how could this happen?

I'd agree with the above. Looking at the pins, there are some marks that look like they've been created to hold the pin in. I have been tempted to do something similar with a rear mech, but it's a massive faff to drill a hole in one end in the right place to allow the pins to be pushed out, then to prevent the pin coming out the same way, having to create those indents to keep it in.

You can tap the pins out with a punch iirc.
This sort of thing is quite common with campagnolo, but shimano offered a better range of component compatibility so rarely needed customising.
It would be worth comparing xt&lx cages to see whether the po was trying to modify to his advantage.
