Found the problem ... after rechecking and confirming I wasn't actually missing any parts I decided to break down the entire hub ... seems whoever had worked on the hub previously had installed the two spacers (16mm & 14mm) in the wrong sequence 
. Will take the opportunity to replace the bearings but hopefully will go back together without issue ...
