so I need 2x 4 and the little locking washer.

No idea if you need the locking washer, it should be with the headset anyway. You can use a normal spacer if needed.

With all this missing, what are the bearing surfaces like?

You need x38 (probably 40 or 42 if you haven't got the cages or they are in poor condition) 1/4" ball bearings.

Pot Luck would find you the plastic angled seals (4), so just stick quality thicker grease in them.

ive counted I would need 36. the cages are alright but I haven't cleaned them thoroughly yet. crown race and cups are good. I just wanted to see if anyone has a spare set.

what I could do is measure the cups and order some sealed bearings.

couldn't I get some seals made?

Are you sure, if it is 36 then it is a 1" headset and not the 1-1/8"

You can measure the part to make sure.
