This what you're looking for? SJS carry a lot of Shimano brake spares so might be worth a look there for what you need.
gios.it is the website for the ones made in the far east, giostorino.it is the website for the original Gios company building custom frames still in Turin.
This previous thread might be of some use if you're trying to fit down tube shifters to a Cannondale.

To demonstrate how they fit, see the photos. Ignore the the broken part of the tab in the first photo as it still has enough thread for the shifter to screw onto.
That's the ones. They fit onto the existing frame mounts and then the shifter fits on top. The photo shows why you can't get the existing frame mounts off as they're screwed through from each side. You have to use a 7mm spanner on the black flats on either the drive side or non drive side to...
All you need are two flat backed washers instead of the curved washers that will fit onto the existing frame shifter mounts to get the shifters to fit. No drilling, cutting or hacking required. I can post up some photos later to explain if needed.
Closest I can see to yours is maybe this from 1978, although could be a year or two earlier. As you say, maybe one or two things may have been changed or replaced over the years such as the rear brake cable route or the forks...
It would look better if you replaced that seat post with a silver one... and sorted out the bar tape gaps around the shifters and put the quick release buttons back in on the levers... and took off the big saddle bag for the photos.
Used FC-RS500 chainset, minor cosmetic marks but still in decent condition. 50/34 chain rings, standard 24mm axle, pedal threads all good.
Looking for £36 posted second class in the U.K. only.
