^ I can confirm you need those flat shifter boss covers. I checked a pic of my old frame and the flats are 90° to yours, but I seem to remember those covers will go on in all 4 positions and I had to move them round to get the shifter in the right position. Is the indexing mechanism in the back of the lever correctly located in the lever itself? Older 6-speed and the new Dura Ace 10-speed ones both go on in the same position so I assume they all must be the same.
