Threaded Headset lock nut and cups starting to look tired

Black paint is now see through and its beginning to rust.
Should I replace the whole headset?

Yes, chances are if the outside has started to rust the inside might be the same. Check for roughness turning the wheel and if necessary take apart and check the bearings and inside faces of the bearing cups for wear/damage. Also rusty parts looks like lack of mainteneance = Lazy
